Document Form Mapping

Add New Document Form Mapping

In Sunrise Patient Portal™administrators can map forms to specific documents using the account maintenance tab.

Before you begin

You must create a document in configuration tools. The created document will be available in Form Document Mapping and can be added to a form. The Document must have the document property set as available to patient and the data collection as patient portal form.

Procedure

  1. Click the Acct Maint tab.
  2. In the Page Menu panel click Form Document Mapping.

    Form Document Mapping is displayed.

  3. Select a Form.
  4. Select a Document.
  5. Click Save.

Results

The form and document are mapped together and are available in the Form Document Mapping window

Edit Existing Form Document Mapping

You can edit existing form document mapping configurations.

Procedure

  1. Click the Acct Maint tab.
  2. In Page Menu panel click Form Document Mapping.

    Form Document Mapping information is displayed.

  3. Click the pencil icon to edit the existing the Form Document Mapping.
  4. Select a new Form or a new Document.
  5. Click Save.

Results

The form and document are edited and saved in Form Document Mapping.

Delete Existing Form Document Mapping

You can delete a previously created form document mapping configuration.

Procedure

  1. Click the Acct Maint tab.
  2. In Page Menu panel click Form Document Mapping.

    Form Document Mapping information is displayed.

  3. To delete a configuration click the red X next to an existing form document mapping.

    The Confirmation window is displayed.

  4. Click OK.

Results

The form document mapping is deleted.
