Immunizations

The Immunizations web part displays the immunizations that have been assigned to you as events by your care provider. If your Sunrise™ Patient Portal account is connected to a personal health record, the Immunizations web part also displays immunizations stored in the personal health record.

The web part displays the following basic information for each immunization:

By default, the immunizations are sorted alphabetically by event name. You can change the sort order by clicking any of the column header links.

To view the most current information, click the Refresh link.

View immunizations

You can choose which immunizations to view by selecting a source and status.

Procedure

  1. For Show records from, select a source from which to display immunizations:
    • Select Sunrise to view the immunizations stored in your patient record.
    • Select Microsoft HealthVault to view the immunizations stored in the personal health record.
  2. For Status, select an immunization status to view:
    • Select All to display all immunizations.
    • Select InComplete to display immunizations that have pending doses.
    • Select Complete to display immunizations for which all doses have been completed.

Results

The immunizations from the selected source and having the selected status are displayed.

Adding Immunizations

You can use Sunrise™ Patient Portal to copy an immunization from your patient record to your personal health record, add a new immunization to a personal health record, or add a new completed dose for an immunization stored in a personal health record.

Procedure

  1. To copy an immunization from your patient record to a personal health record, click Copy to Microsoft HealthVault.

    The immunization and its dosage details are copied from your patient record to your personal health record.

  2. To add a new immunization to a personal health record, from Immunization Details, click Add to PHR.

    Immunizations Details refreshes with blank fields.

  3. Enter the vaccine name.
  4. In the Dose Details section, enter the dosage details for the immunization.
    Note: An immunization must have at least 1 completed dose.
  5. To add the dose information for the immunization, click Add to list.
  6. When you finish adding the details for each completed dose, click Close to close Immunization Details.

Update immunization dose details

You can update the information about an immunization you have stored in a personal health record.

About this task

You cannot update the details of an immunization dose that is stored in your patient record.

Procedure

  1. In the list of immunizations, click the event name. Immunization Details opens.
  2. From the Doses completed table, click the date of the dose that you want to update.

    The dosage details are displayed in the Dose details section of the window.

  3. Make the necessary changes.
  4. Click Update to save your changes.

Delete immunizations

About this task

You can use Sunrise™ Patient Portal to delete immunizations that are stored in a personal health record.

Note: You can only delete immunizations from a personal health record. You cannot delete immunizations from your healthcare provider's records.

Procedure

  1. To delete immunizations stored in a personal health record, click Delete from PHR.

    A confirmation message is displayed.

  2. Click OK.

    The immunization is deleted from the personal health record.

  3. To delete an immunization dose, in the Doses completed table, click the red X for the dose that you want to delete.

    A confirmation message is displayed.

  4. Click OK.

    The immunization dose is removed from the list of completed doses.

Print Immunizations

You can print the immunizations details from the Immunization web part main page.

Procedure

  1. Click the Medical Info tab.
  2. From the left pane, click Immunizations.

    The Event Name, Last-Completed Dose and Location is displayed in immunizations summary.

  3. Select each Event Name.
  4. Click Preview.

    Print Preview is displayed.

  5. Click Print.

Results

The immunization information is printed.
