Health Problem Details

Health Problem Details displays detailed information about the record you are viewing. Information is displayed on 2 tabs: The Detail tab and the Record Info tab.

Detail Tab

The Detail tab displays a comprehensive summary of all the information contained in the record you are viewing, including:
Field Description
Display Name The name of the health problem
Description A descriptive summary of the health problem
Status Designates the health problem as either Active or Inactive
Onset Date The date the health problem began
Resolved Date The date the health problem stopped

Record Info Tab

The Record Info tab displays detailed information about the record you are viewing. While the Detail tab displays a summary of all the information the record contains, the Record Info tab displays information about the record itself, including the source where the record was retrieved and the system where the record was originally created.

Note: A record's information source may differ from its original source. For instance, if your care provider enters a record of a new health problem for you in their computer system, and then you use Sunrise Patient Portal™ to copy the record into your personal health record. When you view your list of health problems, the record will appear as being retrieved from Microsoft® HealthVault™, even though the record was originally created in your care provider’s computer system.

The Record Info tab also includes a table that displays a history of the record, including information about when the record was created and when it was last modified, if applicable. This information includes the application the record was created in, the user who created the record, and the date and time the creation or modification took place.

Add health problems to a personal health record

Use this site to copy a health problem from your patient record to your personal health record or add a new health problem to a personal health record.

About this task

Note: To perform either of the above tasks, your account must be connected to a personal health record.

Procedure

  1. To copy a health problem to your personal health record, select the check box for the health problem, then click Copy to Microsoft HealthVault.

    The health problem is copied from your care provider's records to your personal health record.

  2. To add a new health problem to a personal health record, click Add to PHR.

    The Health Problem Details window appears.

  3. Complete the fields on the Health Problem Details window.
  4. Click Save.

    Health Problem Details closes and the health problem is added to the personal health record.

Update health problem details

About this task

You can update information about a health problem directly in the Health Problem Details window.

Note: You can only update the details for a health problem that is stored in a personal health record. You cannot update the details of a health problem that is stored in your patient record.

Procedure

  1. Update the fields in the Health Problem Details window.
  2. Click Save to save your changes.

    The health problem information is updated and the Health Problem Details window closes..

Delete health problems from a personal health record

You can use this site to delete a health problem that is stored in a personal health record.

About this task

Note: You can only delete health problems from your personal health record. You cannot delete health problems from your care provider's records.

Procedure

  1. Select the check box for the health problem.
  2. Click Delete from PHR.

    A confirmation message is displayed.

  3. Click OK.

    The health problem is deleted from the personal health record.

Print health problems details

Procedure

  1. Click PDF Print Preview.

    File Download opens.

  2. Click Open.

    A PDF Print Preview window opens, displaying the health problem information.

  3. Click Print.

    The Print Options window opens.

  4. Select your print options and then click OK.

    The health problem information is printed.

  5. Close the Print Preview window.
