My Forms Page

Use the My Forms page to view forms that you have worked with.

The My Forms page is composed of the following 3 tabs.

Use the active forms tab

About this task

The Active Forms tab displays all the maintainable forms that you have completed and saved. From this tab, you can open a form, update your information, and send the information to your care provider.

Procedure

  1. Click the Form name.

    The form opens in a new browser window.

  2. Update the information on the form.
  3. Click Save to save your updates.

    The form information is updated and sent to your care provider.

Use the unsubmitted drafts tab

About this task

The Unsubmitted Drafts tab displays the submittable forms that you have begun filling out, but have not yet submitted. Because the forms have not been submitted, the information contained on the forms has not yet been sent to your care provider.

Procedure

  1. Click the form name.

    The form opens in a new window.

  2. Update the information on the form.
  3. When you are finished, do one of the following:
    • Click Save Draft to save a draft of the form without submitting it to your care provider. The form is saved as a draft and the information is not submitted to your care provider.
    • Click Submit to submit the form to your care provider. The information in the form is submitted to your care provider and the information is committed to their system; you cannot update the information on the form. The form no longer appears on the Unsubmitted Drafts tab, but now appears on the Submission History tab.

Use the submission history tab

About this task

The Submission History tab displays a history of all the submittable forms you have submitted. Because the forms appearing on this tab are submittable forms, the information on the forms has been sent to your care provider and cannot be updated. You can however, view forms that you have submitted.

Procedure

Click the form name.

The form opens in a new window.
