The Forms Library displays a list of all the forms that have been published by your care provider. Forms are used by your care provider to collect specific information about you.
Each form opens in a web page that enables you to enter information in fields on the form. When you finish completing a form, you can send your form information to your care provider.
There are 2 types of forms that you can complete: maintainable forms and submittable forms.
Maintainable forms are forms that you can update continuously. This type of form can be viewed as a living document because it remains in a constant state of activity, enabling you to continuously update the information that the form contains. Each time you save a maintainable form, the form is sent to your care provider, however the form remains active and available for you to update.
Maintainable forms only have a Save option. When you save the form, the form information is sent to your care provider, but the form is still able to be updated.
A submittable form is a form you can use to insert information and submit to your care provider After you fill in information, the form is no longer available for you to update.
You can save a submittable form as a draft and come back to it later to finish working. When you save a form as a draft, a temporary version of the form is saved. The draft form is not sent to your care provider. You can return to a draft form at any time to complete the form and submit it to your care provider. Form drafts that you have saved can be accessed from the Unsubmitted Drafts tab on the My Forms web part
Users with administrator rights can export forms from Sunrise Patient Portal™ to make them available to be imported into other Sunrise Portal applications. When you export a form it is saved as a SQL file to your local computer or network. After the file is exported you can import the form in another Sunrise application by executing the SQL file in the database for the application.
