The Document History web part displays a history of Continuity of Care Documents (CCD) and a history of generated clinical documents successfully transferred to a patient and care provider via direct ID or email.
This History tab displays information about the patient successfully transferred history of the Continuity of Care Doucments (CCD) and Generating Clinical Document with below details:
| Fields | Description |

| Document name | Transferred and generated documents name. |
| Action | Transfer or Generate labels are displayed based on patient action. When a patient transfers the document the information in the transfer field is displayed. |
| Transfer Type | When a patient document is transferred successfully the email labels displays. For a successfully generated document a label of N/A displays as the transfer type. |
| Transferred To | Displays the providers' email id used by the successfully transferred document and the patient’s direct ID or email. |
| Time | The time when the successfully transferred and generated document. |
