One of the following:
- Maintenance - Use this type if you wish to create a form
that patients can complete and submit, but still have the
ability to update the form later. This type of form can be
viewed as a "living" form that patients can continuously
update.
- Submission - Use this type if you wish to create a form that
patients complete and submit once. Once submitted, the
information on the form is permanent and cannot be updated
by patients.

The name of the form. The form name will be displayed in the
forms library after the form is published. |

Defines the type of form you are creating and its
purpose. |
| Title |
The title that will be displayed at the top of the form. |
| Foreground Image |
The filename of an image to be used in the foreground, such a
logo. |
| Background Image |
The filename of an image to be used as a background. |
| Default Style |
Displays the Style popup where the user can set the default style
for the whole form. |
