Work with care provider details

You can use Care Provider Details to perform several tasks.

  • View Care Provider details
  • Add Care Providers to a Personal Health Record
  • Update Care Provider details
  • Delete Care Providers
  • Print Care Provider details

View care provider details

Care Provider Detail contains detailed information about the record you are viewing and displays it in two tabs: The Detail tab and the Record Info tab.

Detail Tab

The Detail tab displays a comprehensive summary of all the information contained in the record you are viewing, including:

  • Name
  • Role
  • Address
  • City
  • State
  • Country
  • ZIP Code
  • Phone Number
  • Fax Number
  • Status (Active or Inactive)
  • Effective Date
  • Expiration Date
  • Notes

Record Info Tab

The Record Info tab displays detailed information about the record you are viewing. While the Detail tab displays a summary of all the information the record contains, the Record Info tab displays information about the record itself, including the source from which the record was retrieved and the system where the record was originally created.

Note: A record's information source may differ from its original source. For instance, if your care provider enters a record a new care provider for you in their computer system, and then you use Sunrise Patient Portal™ to copy the record into your personal health record. When you view your list of care providers, the record will appear as being retrieved from Microsoft™ HealthVault™, although the record was originally created in your care provider’s computer system.

The Record Info tab also includes a table that displays a history of the record, including information about when the record was created and when it was last modified, if applicable. This information includes the application the record was created in, the user who created the record, and the date and time the creation or modification took place.

Add care providers to a personal health record

Use Care Provider Details to copy a care provider from your patient record to a personal health record. You can also add a new care provider to a personal health record.

Procedure

  1. To copy a care provider from your patient record to your personal health record, click Copy to Microsoft HealthVault.

    The care provider is copied to the personal health record.

  2. To add a care provider to a personal health record, click Add to PHR.

    Care Provider Detail refreshes with blank fields.

  3. Enter the care provider information.
  4. When finished, click Save.

    The care provider is added to the personal health record.

Update care provider details

About this task

If you are viewing details for a care provider stored in a personal health record, you can update the care provider details directly in the Care Provider Detail window.

Procedure

  1. Click in the desired field and update the information.
  2. When you are finished, click Save.

    The care provider information is updated and saved to the personal health record.

Delete care providers

If you are viewing details for a care provider stored in a personal health record, you can delete the care provider from the personal health record.

Procedure

  1. Click Delete from PHR.

    A confirmation message is displayed.

  2. To delete the care provider, click OK.

    The care provider is deleted from the personal health record.

Print care provider details

Procedure

  1. Click PDF Print Preview.

    File Download opens.

  2. Click Open.

    A PDF Print Preview window opens, displaying the care provider details.

  3. Click Print.

    Print Options opens.

  4. Select your print options and click OK.

    The care provider details are printed.

  5. Close the Print Preview window.
