Fields

A field is a place on a form that is used to collect data entered by a patient or to display a graphical element.

Field Types

A field is a place on a form that is used to collect data entered by a patient or to display a graphical element.

Field Type Description
Calendar Inserts a calendar from which users can select a date.
Check-box Inserts a field that allows users to make multiple selections from a number of options.
Drop-down Inserts a field that allows users to make one selection from a number of options.
Horizontal Line Places a horizontal line on the form.
Hyperlink Inserts a hyperlink on the form.
Image Inserts an image on the form.
Label Use this type if you want the field to display a word or phrase to indicate what follows on the form.
List box Inserts a field that allows users to select one or more items from a list contained within a multiple line text box.
Numeric Inserts a field for collecting numeric information.
Phone Inserts a field for collecting a phone number.
Radio Button Inserts a field that allows users to choose one of a predefined set of options.
Text Inserts a field for collecting textual information.

Fields that are added to a form must be added to a section. A field can be moved up or down in a section using the Up and Down arrows in the component tree. Use the red X to delete a field from a section.

Field attributes

The following table provides a description of the various attributes that can be defined for fields.
Note: The attributes that are displayed depend on the type of field you are adding.
Attribute Description
Column Position Determines which column the field starts in.
Column Span Determines the maximum number of columns the field crosses.
Image The UNC path for an image that you are inserting on the form.
Is Required Determines if the field is required to be completed by patients.
Label The label for the form field.
Line breaks before Determines how many line breaks are inserted before the field.
Name A unique identifier for the field. This value is what will be displayed in the component tree.
Type The kind of form field that you want to add.
Conditional Controls
Condition Defines the field for which conditional branching will apply. The value of the field selected for this property will determine whether the field you are working on is displayed on the form.
Note: The only fields that can be selected for conditional branching are radio buttons and drop-down fields that are in the same section as the field you are working with.
Value Defines the conditional value for the field selected in the Condition property field. If respondents select this value for the field defined in the Condition property field, the field you are working on is displayed on the form. If respondents do not select this value for the field in the Condition field property, the field you are working on is not displayed on the form.
Options
# Lines The number of vertical lines the box has.
Allow decimals Indicates whether decimals are permitted in the field.
Dictionary The dictionary, if any, used to populate the list options.
Layout Determines whether the options are displayed side by side or stacked vertically.
Length The width of the field on the form.
Min length and Max length The fewest and most characters a patient is allowed to enter in the field.
Min value and Max value The lowest and highest values a patient is allowed to enter into the field.
URL The URL of a hyperlink you are inserting on a form.
Default Value Options
Default Selection The value that is automatically selected in a drop-down list prior to a respondent selecting a value.
Default Value The value that is automatically displayed in the field prior to a respondent entering a value. For instance, "Enter your first name in this field".
Prefill from Profile Field Assigns the field a profile term. If a patient has a value saved for the assigned profile term, the value is automatically filled in the field.
Save patient value to profile: Select this check box to overwrite a patient's existing profile term value with the value that the patient enters in the field. For instance, a field where the patient's information may change intermittently and you want the profile term value to save the most up to date value. This prevents the field from being automatically populated with outdated information.

Add fields to a section

Procedure

  1. In the component tree, select the section to add the field to.
  2. On the form editor toolbar, click New Field.
  3. Specify the properties for the field.
  4. To save your work, click Save on the form editor toolbar.

Results

The field is added to the section you specified.

Apply styles to field properties

Certain properties of a field can be formatted to stylize the field according to your preference, such as a field label appearing in bold or a text box displaying a certain background color.

Procedure

  1. Next to the field property, click Style.

    The Style window opens.

  2. Select the style options for the field property.
  3. To apply the style to the field property, click OK.
