Create New Account collects all the information necessary for you to create a Sunrise Patient Portal™ account.
If you have a Secure Health Messaging account, you can link the account to your new Sunrise Patient Portal™ account. When you link the 2 accounts, they share your login information. This enables you to log on either account with the same user name and password.
If you have a Secure Health Messaging account, the application will notify you that a Secure Health Messaging account with the entered user name has been found. If you don't have a Secure Health Messaging account, the system will notify you whether the user name is available.

| Middle name | Enter your middle name. | 
| Date of birth | Click in the field and use the calendar tool to select
                                        your birthdate. Note: If you will be using Sunrise Patient Portal™ to connect to your
                                            patient record in your healthcare organization's system,
                                            you must use the same date of birth contained in your
                                            patient record. | 
| Enter your email address. Note: If you will be using Sunrise Patient Portal™ to connect
                                            to your patient record in your healthcare organization's
                                            system, you must use the same email address contained in
                                            your patient record. | |
| Confirm email | Enter your email address again for confirmation. | 
| Phone | Type your phone number. | 
| Security Question | Select a security question that will be used to verify your identity in the event that you forget your password. | 
| Security Answer | Enter the answer to your selected security question. | 
The system checks to make sure an account with matching details doesn't already exist. If a duplicate account is found, it is displayed in the window.
