Create a new account

Create New Account collects all the information necessary for you to create a Sunrise Patient Portal™ account.

About this task

If you have a Secure Health Messaging account, you can link the account to your new Sunrise Patient Portal™ account. When you link the 2 accounts, they share your login information. This enables you to log on either account with the same user name and password.

Procedure

  1. Do one of the following:
    • If you have a Secure Health Messaging account, type your Secure Health Messaging account user name.
    • If you don't have a Secure Health Messaging account, type a user name for your new Sunrise Patient Portal™ account.
  2. Click Check availability.

    If you have a Secure Health Messaging account, the application will notify you that a Secure Health Messaging account with the entered user name has been found. If you don't have a Secure Health Messaging account, the system will notify you whether the user name is available.

  3. Do one of the following:
    • If you have a Secure Health Messaging account, enter your Secure Health Messaging account password, and then enter it again for Confirm password.
    • If you don't have a Secure Health Messaging account, enter a password for your new Sunrise Patient Portal™ account and then enter it again for Confirm password.
  4. Enter the rest of the applicable information.
    Field Description
    Last name Enter your last name.
    Note: If you will be using Sunrise Patient Portal™ to connect to your patient record in your healthcare organization's system, you must use the same last name contained in your patient record.
    First name Enter your first name.
    Note: If you will be using Sunrise Patient Portal™ to connect to your patient record in your healthcare organization's system, you must use the same first name contained in your patient record.
    Middle name Enter your middle name.
    Date of birth Click in the field and use the calendar tool to select your birthdate.
    Note: If you will be using Sunrise Patient Portal™ to connect to your patient record in your healthcare organization's system, you must use the same date of birth contained in your patient record.
    Email Enter your email address.
    Note: If you will be using Sunrise Patient Portal™ to connect to your patient record in your healthcare organization's system, you must use the same email address contained in your patient record.
    Confirm email Enter your email address again for confirmation.
    Phone Type your phone number.
    Security Question Select a security question that will be used to verify your identity in the event that you forget your password.
    Security Answer Enter the answer to your selected security question.
  5. Click the link to view the terms and conditions.
  6. After you read the terms and conditions, select the check-box to acknowledge your agreement.
  7. Click Create.

    The system checks to make sure an account with matching details doesn't already exist. If a duplicate account is found, it is displayed in the window.

  8. If a duplicate account exists, do one of the following:
    • If the duplicate account is your correct Sunrise Patient Portal™ account and you do not want to create a new one, click the user name for the account. You are returned to the login page with your user name automatically filled in.
    • If you still want to create the new account, click Continue Account Creation to proceed with the creation of the new account.
  9. Your Sunrise Patient Portal™ account is created and a confirmation message appears. The system also sends you an e-mail message containing instructions about how to activate your Sunrise Patient Portal™ account.
